For infestation in the house the best thing is a bug bomb (foggers). Remove yourself, the dog, and all dog toys and your toothbrush form the house. Open all cabinets, drawers, and interior doors. You will need 1 can for every 500 or so square feet (this can be variable depending on the brand). Set them off and leave. While the house is uninhabitable (2-4 hours depending on the brand, the longer the better) bathe the animals with a flea shampoo and treat with program, avdantage, frontline or other flea killer repelant. When you can go back in you will need to wash all linens, doggy beds, dishes, silverware, cookware and the like to remove the residues.

They also have foggers/bug bombs you can use in yard. We use both the indoor and outdoor varities about once every 3 months as prevention. And as long as you wait the specified time and clean appropriately afterward there is no danger to you or you pet. The toxin only stays active for the time you are required to stay out of the house, after that it becomes inert.
