I know this is going to sound completely out of context, but I read the article/post mulroony shared, and just want to say that "burows solution" mentioned in the latter part works VERY well on diaper rash in kids, has little to no odor, and no side effects (on the kids anyway)
considering the similarities (irritation, inflamation, need to dry, etc) this might be something to ask the vet about using

The first step in treating a hot spot is to get it dry. all the fur covering and immediately surrounding the lesion must be clipped or shaved. A month ago, My goldern has a hot spot on the side of his face below the ear. The vet shave his hair on the affected area. The vet told me to clean the area with Antiseptic solutions, allow it to dry, then put cream on twice a day. He recovered in 3 weeks.
Do not use alcohol to keep it dry, rubbing alcohol is too irritating.
If you cant reallly afford to take him to the vet, can you ring up the vet, ask him to sell you an Antiseptic cream for hot spot ?!
Some severe hot spot need oral medicine.

When my rottie had a hot spot I had to clean it twice a day with an antibacterial soap and keep it dry. I didn't think to use cream but that sounds like a great idea.

Right age and right breed for demodex. Are you sure they are hotspots, because thats what I thought cooper had, and thats what my friend thought her pitbull had, but both (5 month old) pups had demodex. All the hotspot meds in the world isnt going to make demo go away!
